Hi,

Thank you for your interest in an individual dental plan. As an individual there is a rich plan I can offer you through Empire Blue Cross Blue Shield called Prime C.

How much will it cost? It depends on your home zipcode, date-of-birth and coverage type (self, couple or family). In order to quote, please supply these three items.

Attached are the enrollment form, schedule of benefits and Reg 194 that you must review, complete and sign. The "Prime Plan C" is a PPO which allows you to see wither an in-network provider or any dentist of your choice. A more detailed plan document is available upon request.

The individual plan does not offer an orthodontic benefit and if you visit an in-network provider you would not be eligible for the negotiated rate.
